Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide In-Home Health Evaluations. You will engage with people in the comfort of their own homes, traveling up to 100% of the time across a defined coverage area.

You’ll bridge gaps in care and perform evaluations using an iPad with a streamlined workflow. You must hold an MD or DO with active licensure, be able to obtain multi-state licenses, and collaborate with a national clinician network.

Qualifications

• MD/DO with active/unrestricted license(s) in coverage areas.
• Able to obtain and maintain multi-state licenses as required.

• Be part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs.
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products.
• Be flexible to travel locally and within licensed states.
