Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy or PT-related master’s with PT bachelor.
• Eligible for clinical supervision and home-based PT delivery.

Responsibilities

• Provide in-home outpatient rehabilitation to older adults.
• Assess and develop PT plans and coordinate referrals.
• Supervise PT Assistants and students where allowed.
• Deliver patient and caregiver education for independence.
• Maintain documentation and ensure safety in home settings.
