VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in the home, helping patients regain mobility and independence. You'll work autonomously as part of a compassionate care team.

You'll evaluate, plan, and implement therapy, educate families, document progress in the EMR, and collaborate with nurses and physicians. Texas-based opportunities with flexible scheduling and growth.

Current PT license in Texas required; 1–2 years home health preferred.

Formación

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice (Texas).
• Minimum 1–2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsabilidades

• Evaluate and assess patients’ home-based functional status, mobility, strength and rehabilitation potential.
• Develop and implement individualized home health care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence in the home.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Document patient care and progress accurately in the EMR and participate in team meetings.
