Job summary

Twin Cities Physicians is seeking a full-time Wound Care Nurse Practitioner to lead advanced wound assessments and management across assisted living, memory care, and skilled nursing facilities in the Twin Cities Metro. You’ll coordinate care with physicians, nurses, and facility leadership, focusing on complex wounds and evidence-based interventions.

Eligible candidates hold an AGNP or Minnesota NP license, with strong wound care or geriatrics background.

Qualifications

• Active Minnesota NP license with AGNP preferred.
• Experience in wound care, geriatrics, or post-acute settings.
• Strong clinical judgment for medically complex patients.

Responsibilities

• Provide advanced wound care assessments and treatment planning for complex wounds.
• Conduct routine rounding visits across senior living communities.
• Manage medications, diagnostics, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
• Identify barriers to healing and implement evidence-based interventions.
• Support continuity of care and improve clinical outcomes.
