Bay Area Community Services is seeking a Peer Counselor for the CalMHSA Medi-Cal Peer Support Specialist role within the Community Wellness Response Team in Sacramento. The position emphasizes field-based peer support, crisis response, and helping participants toward recovery using lived experience.

Responsibilities include outreach, ongoing peer visits, implementing care plans, documenting progress, and coordinating with a multidisciplinary team.

Qualifications

• Peer certification required. CalMHSA Medi-Cal Peer Support Specialist or equivalent.
• Peer support experience required.
• High school diploma required.
• Use of a computer or electronic platform to document work is required.
• Bilingual in threshold language preferred.
• Experience navigating the social services system preferred.
• Experience working in various community settings.

Responsibilities

• Provides field-based peer support and counseling to participants and families.
• Performs outreach to locate potential participants and referrals.
• Implements plan of care, offers crisis support, and links to services.
• Documents services in progress notes and maintains caseload.
• Collaborates with interdisciplinary team and meets productivity guidelines.
• Maintains calendar of appointments and coordinates with providers.
