Job summary
Bay Area Community Services is seeking Clinical Care Coordinators for the Community Wellness and Response Team in Sacramento. CWRT provides urgent mobile crisis response in partnership with 988 and 911, operating 24/7 with two-person teams in company vehicles to de-escalate crises and connect clients to resources.
The role includes field counseling, risk assessment, service planning, and collaboration with county responders.
Qualifications
- Master’s Social Work (MSW), Masters in Marriage & Family Therapy (MFT) or MPCC required.
- Registered with BBS as Associate ASW/LCSW or Associate MFT/LMFT.
- 2 years of social service experience required.
- Valid California driver’s license and access to a personal vehicle.
Responsibilities
- Disptaching and response for crises referred by 911/988 for mobile crisis response.
- Coordinate with mobile teams, dispatch, and law enforcement to deescalate crises.
- Provide counseling and case management in the field and develop service plans.
- Assess risk status and intervene appropriately; document data per policy.
- Engage community resources and report disengagement to the team.
- Participate in supervision, meetings, travel, and emergency coverage.
