Job summary

Easterseals PORT Health is seeking a Mobile Crisis Coordinator for the VA REACH Team serving Central Virginia and the Shenandoah Valley. The role focuses on crisis assessment, safety planning, and short-term interventions in community settings.

You will work with families, providers, and multidisciplinary teams to coordinate care, track outcomes, and connect clients with needed resources. The position requires licensure-eligible credentials and a drive to improve community mental health outcomes.

Qualifications

• Master's degree in a license-eligible counseling, social work, marriage and family therapy, or related clinical field.
• REQUIRED: License-eligible as an LPC, LCSW, or LMFT in Virginia.
• Minimum of one year of experience working with mental health populations preferred.
• Strong crisis intervention, adaptability, critical thinking, problem-solving, and decision-making skills.
• Experience collaborating within multidisciplinary treatment teams preferred.
• Proficiency with Electronic Health Records (EHR), Microsoft Office Suite, and related technology platforms.
• Valid driver's license, acceptable driving record, current auto insurance, and ability to travel throughout the service area.

Responsibilities

• Provide mobile crisis assessments, interventions, stabilization services, and short-term counseling in community settings.
• Partner with individuals, families, healthcare providers, first responders, and community stakeholders to support crisis resolution and recovery.
• Coordinate treatment recommendations, referrals, service plans, and follow-up care.
• Document assessments, treatment plans, interventions, and client outcomes in accordance with regulatory and agency requirements.
• Facilitate support meetings and collaborate with multidisciplinary teams to ensure comprehensive, person-centered care.
• Contribute to quality improvement efforts and positive client outcomes through effective crisis response and care coordination.
• Participate in the program's rotating on-call schedule as assigned.
