Easterseals PORT Health in Staunton, VA seeks a REACH Mobile Prevention Coordinator to provide proactive, in-home and community-based crisis prevention and coordination services for individuals with intellectual and developmental disabilities and co-occurring behavioral health needs.

You’ll collaborate with families, providers, case managers, and community partners to develop proactive support plans, coordinate services, and promote stability and successful community living, including telehealth

Qualifikationen

• QMHP/QDDP credentials or eligibility are required.
• Bachelor’s degree in Human Services or related field is required.
• Master’s degree in Counseling, Social Work, Psychology, or related field preferred.

Aufgaben

• Provide mobile prevention services for individuals with I/DD and behavioral health needs.
• Implement crisis prevention strategies in-home, community, phone, and telehealth settings.
• Coordinate support meetings and develop Crisis Education and Prevention Plans.
• Collaborate with case managers, providers, families, CSBs and community partners.
• Identify training and technical assistance needs for providers and caregivers.
• Coordinate admissions/discharges for REACH Mobile Supports and Crisis Therapeutic Home services.
• Maintain relationships with community agencies and referral partners.
• Complete documentation and reporting within established timeframes.
• Participate in a shared 24/7/365 on-call rotation and crisis response as required.
• Ensure compliance with HIPAA, DBHDS regulations and Medicaid/Medicare requirements.

Ausbildung

Bachelor's degree in Human Services or related field

Master's degree in Counseling, Social Work, Psychology or related field
