Job summary

Easterseals PORT Health in Staunton, Virginia seeks a REACH Mobile Prevention Coordinator to join our Virginia REACH Team. You will provide mobile prevention and crisis planning for individuals with intellectual and developmental disabilities and co-occurring behavioral health needs, supporting families and partners.

The role requires QMHP or QDDP certification, a Bachelor’s degree, and experience; a Master’s degree is preferred.

Qualifications

• Bachelor's degree required in human services or related field.
• Master's degree preferred in counseling, social work, psychology, or related behavioral health field.
• QMHP and/or QDDP certification required.
• Virginia license (LPC/LCSW/LMFT) preferred.

Responsibilities

• Provide mobile prevention services for individuals with intellectual and developmental disabilities and co-occurring behavioral health needs.
• Implement crisis prevention strategies through in-home, community-based, phone, and telehealth support.
• Coordinate support meetings and develop Crisis Education and Prevention Plans.
• Collaborate with case managers, Clinical Home providers, CSBs, families, and community partners to ensure coordinated care.
• Identify and coordinate training and technical assistance needs for providers, caregivers, and natural supports.
• Coordinate admissions and discharges for REACH Mobile Supports and Crisis Therapeutic Home services.
• Maintain strong relationships with community agencies and referral partners.
• Complete documentation and reporting within established timeframes.
• Participate in 24/7/365 on-call rotation and provide crisis response as required.
• Ensure compliance with agency policies, HIPAA, and Medicaid/Medicare requirements.
