Easterseals PORT Health seeks a REACH Mobile Prevention Coordinator to serve Staunton, VA, providing crisis prevention and coordination for individuals with I/DD and behavioral health needs. Join a Virginia REACH team delivering proactive supports and crisis planning.

The role emphasizes collaboration with families, providers, and community partners, along with documentation, on-call crisis response, and adherence to HIPAA and state regulations.

Qualifications

• Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QDDP) required
• Bachelor's degree in Human Services or related field from an accredited institution
• Minimum of one year of relevant full-time experience required
• Master's degree in Counseling, Social Work, Psychology, or related behavioral health preferred
• Current, unrestricted Virginia LPC, LCSW, or LMFT license preferred
• Experience with individuals with I/DD and behavioral health needs preferred
• Knowledge of Virginia mental health and developmental disability systems of care
• Strong verbal and written communication skills

Responsibilities

• Provide mobile prevention services for individuals with intellectual and developmental disabilities and co-occurring behavioral health needs.
• Implement crisis prevention strategies through in-home, community-based, phone, and telehealth support.
• Coordinate support meetings and develop Crisis Education and Prevention Plans.
• Collaborate with case managers, Clinical Home providers, CSBs, families, and community partners to ensure coordinated care.
• Identify and coordinate training and technical assistance needs for providers, caregivers, and natural supports.
• Coordinate admissions and discharges for REACH Mobile Supports and Crisis Therapeutic Home services.
• Maintain strong relationships with community agencies and referral partners.
• Complete documentation, service records, data collection, and reporting within established timeframes.
• Participate in a shared 24/7/365 on-call rotation and provide crisis response as required.
• Ensure compliance with agency policies, HIPAA, DBHDS regulations, and Medicaid/Medicare requirements.
