Job summary

Easterseals PORT Health in Charlottesville, VA seeks a Mobile Crisis Coordinator to join our VA REACH Team serving Central Virginia and the Shenandoah Valley. You’ll respond to crises, perform assessments, and develop treatment and safety plans.

This role involves collaboration with families, providers, and multidisciplinary teams, conducting short-term interventions, coordinating care, and supporting seamless community-based crisis response with rotating on-call duties.

Qualifications

• Master’s degree in a license-eligible counseling, social work, marriage and family therapy, or related clinical field.
• REQUIRED: License-eligible as an LPC, LCSW, or LMFT in Virginia.
• Minimum of one year of experience working with mental health populations preferred.
• Strong crisis intervention, adaptability, critical thinking, problem-solving, and decision-making skills.
• Experience collaborating within multidisciplinary treatment teams preferred.
• Proficiency with Electronic Health Records (EHR), Microsoft Office Suite, and related technology platforms.
• Valid driver’s license, acceptable driving record, current auto insurance, and ability to travel throughout the service area.

Responsibilities

• Provide mobile crisis assessments, interventions, stabilization services, and short-term counseling in community settings.
• Partner with individuals, families, healthcare providers, first responders, and community stakeholders to support crisis resolution and recovery.
• Coordinate treatment recommendations, referrals, service plans, and follow-up care.
• Document assessments, treatment plans, interventions, and client outcomes in accordance with regulatory and agency requirements.
• Facilitate support meetings and collaborate with multidisciplinary teams to ensure comprehensive, person-centered care.
• Contribute to quality improvement efforts and positive client outcomes through effective crisis response and care coordination.
• Participate in the program’s rotating on-call schedule as assigned.
