Job summary

Qualifications

• High School Diploma with 2 years of experience or a Bachelor’s with 1 year, or a Master’s in social work with internship plus six or more months experience in Case management.
• Fluent English required; strong communication and interpersonal skills.
• Must read, speak and write fluent English; ability to document clearly.
• Proficiency in Word processing and Excel for reporting and charts.
• Valid California driver’s license and access to reliable vehicle.

Responsibilities

• Provide client-centered case management in a mobile wellness unit across LA and Ventura counties.
• Assess eligibility and needs; link clients to housing and social services.
• Complete timely documentation in accordance with program policies.
• Maintain client confidentiality and HIPAA compliance in all settings.
• Attend meetings, submit reports, and provide back-up support as needed.
