You will work with a small mobile team, assess needs, develop individualized plans, coordinate services, and maintain confidential records across sites.

The role emphasizes outreach to houseless individuals, crisis de-escalation, and collaboration with community partners to empower clients toward lasting transformation and stability.

Qualifikationen

• Bachelor’s degree in Social Work, Human Services, Psychology, Behavioral Sciences, or related field, or equivalent education/experience.
• Experience providing case management, outreach, or direct services to individuals experiencing homelessness or complex needs.
• Knowledge of trauma-informed care and ability to engage participants with empathy and professional boundaries.
• Ability to conduct assessments and develop individualized service plans.
• Experience coordinating referrals and collaborating with internal teams and community partners.
• Strong documentation skills, including maintaining confidential records (HMIS).
• Strong communication, organizational, and crisis de-escalation skills.

Aufgaben

• Conduct outreach to houseless individuals, building relationships and assessing needs.
• Provide participant-centered, trauma-informed case management.
• Complete intakes, assessments, and identify barriers and goals.
• Develop and maintain individualized service or case plans.
• Manage an active caseload with consistent follow-up and support.
• Connect participants to housing resources and stability pathways.
• Coordinate referrals to healthcare, behavioral health, employment, and social services.
• Collaborate with staff, volunteers, and community partners for continuity of care.
• Maintain accurate, timely, and confidential documentation in required systems.
• Respond to crises appropriately using de-escalation and support connections.
