Job summary

Directions for Living is seeking an Outreach Team Member to deliver trauma-informed behavioral health case management to individuals experiencing street homelessness. You will support recovery, strengthen social supports, and empower clients through skill-building and safety planning.

This field-based role connects clients with mental health and housing services, coordinates with partners, and requires dual data entry into EHR and P-HMIS. Flexible hours and community commitment are expected.

Qualifications

• Master’s degree or Bachelor’s in psychology, social work or related field.
• Five years in behavioral health, outreach, crisis intervention or case management.
• Knowledge of community resources; experience with vulnerable populations.

Responsibilities

• Engage individuals experiencing street homelessness through trauma-informed support.
• Deliver case management to foster recovery, safety and personal empowerment.
• Assist with housing options via vulnerability assessments and shelter evaluations.
• Coordinate with service providers and funders to deliver comprehensive services.
• Complete dual data entry in the agency's EHR and P-HMIS systems.
