Job summary

Mission Prep Healthcare, part of the AMFM Healthcare family, seeks compassionate Community Coordinators to support teens 12–17 at Residential Treatment Facilities. The role involves coordination, care, and documentation with a focus on safety and wellness in a team environment.

Compensation is $19.00–$23.00 per hour, with evening shifts Tue–Sat or Sun–Thu. Full-time benefits include medical, dental, vision, 401k, PTO, and education reimbursement.

Qualifications

• Education: High School Diploma or GED; Bachelor's preferred.
• Background checks required (ADP Selective Screening Services and other checks).
• Valid driver's license with assumptions per company policy.

Responsibilities

• Coordinate and transport clients to daily activities ensuring on-time arrivals.
• Use intervention and communication skills with clients and families daily.
• Adhere to Joint Commission policies and document medications and care plans.
• Maintain clean, safe residences and support overall wellness for adolescents.
• Document client activities in the electronic health record (BestNotes).
• Collaborate in a multidisciplinary team to ensure individualized care plans.
