Job summary

ERC Pathlight in Illinois seeks a Program Coordinator to provide patient supervision across inpatient, residential, and partial hospitalization settings. You will support mealtime needs, document care, and assist with therapeutic activities, working across multiple sites as required.

The role requires High School Diploma or GED; valid driver’s license; state credential where applicable; and ServSafe certification depending on location.

Qualifications

• High School Diploma or GED required.
• Minimum one year direct patient care experience (PC II).
• Washington credential may be pending at time of hire.
• ServSafe certification may be required depending on state/licensing.
• Mental Health/Psych Tech or CNA certification preferred.

Responsibilities

• Provide patient supervision and support across inpatient, residential, and partial hospitalization.
• Maintain daily program structure and follow the scheduled plan.
• Assist with mealtime support and coaching for meals and snacks.
• Document observations and events in the EHR for group activities.
• Transport patients to medical appointments as needed.
• Support safety checks, de-escalation, and coping skill practice.

Education

Bachelor’s degree in behavioral health or related field

ServSafe certification
