Job summary

Zumbro Valley Health Center is seeking a Mental Health Practitioner to provide direct mental health services to adults in the community, at home, or at the office. You will collaborate with a dedicated team to ensure high-quality care.

Responsibilities include maintaining a caseload, developing treatment plans with the team, and facilitating individual and group sessions. Experience with severe emotional disturbances is a plus; valid MN license is required for eligibility.

Qualifications

• Bachelor’s degree in Social Work, Psychology or related human services field.

Responsibilities

• Maintain a caseload of clients.
• Develop and collaborate on treatment plans.

• Coach, educate, and model independent living skills.
• Coordinate referrals as needed.
