Job summary

Greenbrook, a clinic-based healthcare provider, seeks a Case Manager to partner with patients throughout treatment in Glen Allen, VA. You will coordinate care, support insurance processes, and ensure a seamless, compassionate patient experience in a fast-paced environment.

Responsibilities include guiding patients from consultation to treatment completion, managing authorizations, and maintaining thorough documentation in CRM/EHR. Strong organizational and communication skills are essential.

Qualifications

• High school diploma or equivalent; 3–5 years in clinical case management or related healthcare setting.
• Experience using EHR systems and Microsoft Office apps (Outlook, Word, Excel).
• Excellent communication, organizational, and customer service skills.
• Preferred: associate/bachelor’s degree in healthcare field.
• Preferred: Salesforce CRM experience and knowledge of insurance processes.

Responsibilities

• Guide patients through their treatment journey from initial consultation through completion.
• Coordinate appointments, provider schedules, prior authorizations, and specialty pharmacy activities.
• Monitor patient progress, address barriers to care, and keep plans on track.
• Maintain accurate patient records and documentation in CRM and EHR.
• Respond to patient inquiries with empathy and urgency while ensuring a stellar patient experience.
• Support positive patient outcomes while ensuring compliance with requirements.
