• 3–5 years of clinical case management or patient care coordination experience.
• Experience with EHR systems and Microsoft Office.

Responsibilities

• Coordinate appointments, provider schedules, authorizations, specialty pharmacy activities, and readiness requirements.
• Monitor patient progress and address barriers to care with cross-functional teams.
