Qualifications

• 3-5 years of clinical case management or care coordination experience.
• Experience with EHR systems and Microsoft Office applications.
• Strong communication, organizational, and patient service skills.

Responsibilities

• Guide patients through their treatment journey from consultation to completion.
• Coordinate appointments, provider schedules, authorizations, and treatment readiness.
• Monitor progress, address barriers, and collaborate with cross-functional teams.
• Maintain accurate documentation and patient records in CRM/EHR systems.
• Respond to inquiries with empathy, professionalism, and urgency.
• Support high-quality outcomes while ensuring compliance with requirements.
