Job summary

Greenbrook is seeking a compassionate Case Manager to guide patients through TMS and Spravato treatment pathways in a clinic-based role in Glen Allen, VA. You will coordinate care, manage authorizations, and support enrollment and scheduling, ensuring a seamless patient experience.

Qualifications

• High school diploma + 3-5 years in clinical case management or related healthcare setting.
• Experience using EHR systems and Microsoft Office (Outlook, Word, Excel).
• Strong communication, organizational, and customer service skills with ability to manage multiple priorities.

Responsibilities

• Guide patients through their treatment journey from initial consultation to completion.
• Coordinate appointments, provider schedules, prior authorizations, specialty pharmacy activities, and treatment readiness requirements.
• Monitor patient progress, address barriers to care, and collaborate with cross-functional teams to keep treatment plans on track.
