Job summary

RWJBarnabas Health is seeking a Case Manager I at Monmouth Medical Center in Long Branch, NJ to coordinate care for patients with severe or persistent mental health needs. You will function within a multidisciplinary team to facilitate treatment planning, advocacy, and seamless transitions across inpatient and outpatient settings.

This Day shift, full-time role emphasizes crisis de-escalation, linkage to housing and social services, accurate documentation, and active participation in team

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, or related field.
• 1–2 years of experience with severe or persistent mental health issues preferred.
• Experience in crisis de-escalation and maintaining professional boundaries.
• Knowledge of local community resources; group facilitation preferred.
• Scheduling: Full-Time, Day Shift (8:00 AM - 4:30 PM, Mon–Fri).

Responsibilities

• Coordinate patient transitions between inpatient units and outpatient clinics.
• Provide resource navigation and psychoeducational materials for discharge.
• Link patients to housing, social services, and health providers.
• Lead structured groups focusing on community resources and planning.
• Document medical records meeting regulatory standards and deadlines.
• Prepare for and contribute to interdisciplinary treatment team meetings.
• Participate in in-service education and ongoing training.
• Assist in planning professional development and training content.
• Provide crisis de-escalation in coordination with the team.
