Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, or closely related human services field required.
• 1–2 years of experience working with individuals with severe or persistent mental health and behavioral health issues preferred.
• Proven experience in crisis de-escalation, maintaining professional boundaries, managing behavioral health documentation, and multidisciplinary communication.
• Knowledge of local community resources and prior Group Facilitation experience running therapeutic or structured skill-based groups are highly preferred.
• Scheduling Requirements: Full-Time, Day Shift (8:00 AM – 4:30 PM, Monday – Friday).

Responsibilities

• Manage smooth patient transitions and communication loops between PESS, inpatient units, and outpatient clinics.
• Deliver targeted resource navigation and prepare or present clear, accessible psychoeducational and mental health materials to patients immediately before and after inpatient discharge.
• Facilitate direct referrals to secure external housing, social services, addiction treatment, and physical health providers while linking patients with needed services.
• Facilitate structured, skill-based groups for mental health patients, focusing on community resources and transition planning.
• Author accurate, thorough medical records and behavioral health documentation meeting internal policy guidelines and regulatory standards.
• Prepare for and actively contribute to interdisciplinary treatment team meetings to optimize patient outcomes.
• Attend specialized in-service education and contribute to training content when required.
• Provide active crisis de-escalation and intervention with the treatment team.
• Execute varying professional tasks and operational duties as assigned.
