Job summary

RWJBarnabas Health seeks a Social Work Case Manager for Jersey City Medical Center to provide psychosocial assessments, discharge planning, and care coordination across the healthcare continuum. You will work with physicians, nurses, and the care team to address barriers to safe discharge and support families through transitions.

The role requires an MSW, NJ LSW/LCSW licensure, and EARC certification within six months, plus 2+ years post-licensure healthcare experience.

Qualifications

• MSW degree required and NJ licensure as LSW or LCSW
• EARC certification within six months of hire
• Two+ years post-licensure healthcare experience
• Experience with discharge planning and care coordination
• Knowledge of CMS, HIPAA, and regulatory requirements
• Strong communication, collaboration, and documentation skills
• Certification in case management preferred (ACM/CCM)

Responsibilities

• Conduct psychosocial assessments and document patient needs, risks, strengths, and discharge considerations in the electronic health record
• Develop and implement clinical interventions to support safe discharge and reduce readmissions
• Participate in interdisciplinary rounds, presenting cases and updating discharge planning
• Assess Social Determinants of Health (SDOH) and coordinate appropriate resources
• Coordinate referrals for post-acute services
• Identify discharge barriers and manage multiple priorities in a fast-paced environment
• Collaborate with Utilization Review for authorization and clinical documentation
• Provide evidence-based psychosocial interventions including counseling and crisis intervention
• Maintain compliant documentation per CMS, HIPAA, and policies
• Participate in quality improvement and department initiatives
• Provide empathetic, culturally sensitive care during transitions
