This fast-paced, collaborative role requires clear documentation and strong communication with patients, families, and care teams to ensure timely, compassionate care.

You will develop discharge plans, gather relevant medical records, and connect patients with community resources to support aftercare.

Qualifications

• High School Diploma or equivalent; Bachelor's Degree preferred in psychology or related field.
• Current CPR certification required or certification obtained prior to start date.
• Experience in a mental healthcare hospital environment preferred.

Responsibilities

• Oversee patient admissions and services, including maintaining medical and legal documents.
• Develop individualized discharge plans with the treatment team and coordinate aftercare with community resources.
• Coordinate communication with families and community providers to support aftercare needs.
• Engage with patients and staff to ensure compassionate, patient-centered care.
