Responsibilities

• Coordinate resources and services for clients and assist clinicians in planning the range of care.
• Advocate for clients and maintain relationships with community groups.
• Complete documentation in case records and maintain caseload as required.
• Visit clients in their homes and community to assess needs and deliver services.
• Provide crisis intervention to support client safety within scope.
• Develop and deliver educational or support groups in the community.
• Serve as liaison with other community agencies and schools.

Education
