Peak Behavioral Health seeks a Case Manager to support the clinical services department by coordinating discharge and aftercare planning for patients. You will partner with the treatment team, families, and referral sources to arrange appropriate services and placement, while maintaining detailed clinical records.

The role emphasizes collaboration with the care team to ensure ISP implementation, timely referrals, and thorough documentation in the patient’s record.

Qualifications

• Bachelor’s degree in a behavioral health area or related field.
• One or more years of related behavioral health experience.
• Prior case management experience preferred.

Responsabilités

• Participates in the development of the service plan.
• Coordinates discharge planning and aftercare arrangements.
• Maintains comprehensive clinical records and coordinates records with providers.
• Connects patients with community resources and referrals.
• Engages patients and families to support aftercare planning.
• Documents all care activities and coordination within timeframes.
