Balance Treatment Center in California seeks a Clinical Case Manager to coordinate care for clients and families, enhance communication among therapists, psychiatrists, and external providers, and ensure treatment plans align with clinical goals.

You will work closely with the treatment team to maintain client well‑being, document care coordination in the EMR, and support discharge planning while helping families navigate the treatment process.

Qualifications

• Bachelor’s or Master’s degree in psychology, social work, counseling or related behavioral health field.
• Strong organizational and communication skills.
• Ability to collaborate with multidisciplinary clinical teams.

Responsabilités

• Maintain the well-being of clients and the integrity of the program.
• Coordinate care between therapists, psychiatrists, families, schools and external providers.
• Serve as a primary point of contact for families regarding care coordination and treatment logistics.
• Support communication between Balance Treatment Center and referring providers and community partners.
• Facilitate coordination of psychiatric appointments and external providers when appropriate.
• Participate in multidisciplinary treatment team meetings to review client progress.
• Support therapists in tracking treatment plan goals and client progress.
• Document care coordination and ensure documentation meets clinical and payer requirements.
• Maintain structured and organized treatment experiences for clients and families.
• Support program leadership in maintaining organized clinical records.
