Clinica Sierra Vista is seeking a BH Case Manager in Bakersfield, CA. The role involves coordinating resources and services for clients, supporting mental health clinicians, and advocating for clients within a multidisciplinary team.

You will visit clients at home and in the community, maintain case records, and contribute to planning care. A Bachelor’s degree in a related field is required, along with a clean drug screen and DMV background check.

Qualificações

• Bachelor’s degree from an accredited college or university in Psychology, Sociology, Human Services, Behavioral Science, Social Work, or related field.

Responsabilidades

• Coordinate resources and services for clients under the supervision of the Clinic Manager.
• Assist mental health clinicians in the development and delivery of services to clients.
• Maintain assigned caseload and client contacts as required by contract requirements.
• Provide crisis intervention counseling at a level not requiring licensure.
• Visit clients in their homes and community to assess needs and determine additional services.
