Job summary

Crestwood Behavioral Health Inc. is seeking a Service Coordinator (Mental Health Case Manager) in Bakersfield, CA to assess clients and coordinate treatment plans within an interdisciplinary team.

This role supports discharge planning and collaborates with nursing staff to ensure continuity of care in a crisis residential setting. Minimum 2+ years in healthcare with mental health experience and licensure with BBS as AMFT/ASW/LPCC are required.

Qualifications

• 2+ years in a healthcare setting, with at least 1 year in mental health.
• Registered with the BBS as AMFT, ASW, or LPCC.
• Knowledge of DSM-5 and psychosocial rehabilitation principles.

Responsibilities

• Assess clients to identify problem areas requiring treatment and complete assessments.
• Coordinate interdisciplinary team meetings and discharge planning.
• Ensure continuity of treatment by coordinating the overall treatment plan with nursing staff.
