Job summary

TURN in San Diego, CA, seeks a dedicated behavioral health case manager to support access to medical, educational, social, housing, and other services. This role emphasizes whole-person care and coordination with physical health providers.

You will perform crisis intervention, discharge planning, and group/individual counseling, documenting progress and maintaining client confidentiality in line with program goals.

Qualifications

• Bachelor's degree in a behavioral science and at least four years of experience providing services to individuals with mental illness.
• Three years of experience providing case management to individuals with mental illness and/or substance use disorders preferred.
• Valid CA driver's license and current auto insurance per contract requirements required.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
