Job summary

Care Initiatives seeks a Memory Program Coordinator (CCDI) to lead memory care programming across more than 40 Iowa skilled nursing communities. Plan, implement, and evaluate activities for residents with dementia, and partner with nursing, social services, therapy, and activities teams to support individualized care plans.

This role requires experience with memory impairment, a Bachelor’s degree in Therapeutic Recreation, Nursing, or Social Work (CTRS preferred), and knowledge of long-term care

Qualifications

• Experience working with residents or patients with memory or cognitive impairment.
• Bachelor’s degree with concentration in Therapeutic Recreation, Nursing, or Social Work preferred.
• CTRS credential preferred.
• Working knowledge of federal and state regulations governing long-term care.
• Strong commitment to empathy, respect, patience, and discretion.

Responsibilities

• Plan, implement, and evaluate memory care programming for residents with dementia.
• Provide person-centered interventions to promote engagement and quality of life.
• Collaborate with nursing, social services, therapy, and activities teams on individualized care plans.
• Monitor responses to programming and adjust approaches as needed.
• Educate staff on best practices for dementia care and communication.
• Support families through education, resources, and participation in care planning.
