Job summary

Care Initiatives is seeking a Memory Program Coordinator (CCDI) to lead memory care programming across our Iowa communities. You will plan, implement, and evaluate activities that support residents with dementia, promoting engagement, independence and quality of life.

You will partner with nursing, social services, therapy, and activities teams to tailor care plans, educate staff on best practices, and monitor outcomes to ensure compliance with state and federal guidelines.

Qualifications

• Experience with residents with memory or cognitive impairment.
• Bachelor's degree with concentration in Therapeutic Recreation, Nursing, or Social Work preferred.
• CTRS credential preferred.
• Knowledge of federal and state long-term care regulations.
• Strong empathy and discretion.

Responsibilities

• Plan, implement, and evaluate memory care programming.
• Provide person-centered interventions to promote engagement and quality of life.
• Collaborate with nursing, social services, and therapies teams to support care plans.
• Monitor responses and adjust approaches for cognitive/behavioral needs.
• Educate staff on dementia care best practices.
• Support families through education and care planning.
• Ensure programs and documentation meet regulatory standards.
• Track participation and program outcomes.
