Qualifications

• Active RN license in Illinois.
• Experience in memory care or dementia care.
• Strong communication and teamwork skills.

Responsibilities

• Support a holistic memory care program.
• Supervise Memory Care Aides and CNAs.
• Document care and maintain records.
• Assist with ADL care and transportation as needed.
• Engage residents in non-scheduled activities.
• Encourage family involvement and communication.
