Job summary

Qualifications

• Two years’ college in a health care field or activities professional by a recognized accrediting body.
• One to two years’ full-time work experience in an activities program in an adult care setting, or similarly related memory care experience.
• CPR and First Aid certification are required.
• Universal Assessment Instrument & ISP certificates or willingness to obtain.
• Must have a thorough knowledge of, social and psychological needs of residents.
• Must have a valid driver’s license and driving record acceptable for company insurance.

Responsibilities

• Designs a creative activity program based on company standards to meet the individual needs and interests of the residents.
• Work closely with the Resident Care Director on a daily basis to better serve residents, families, visitors and staff.
• Monitors day-to-day operation of the Sweet Memories Neighborhood and Assisted Living activity programs.
• Follow state-mandated Assisted Living Facility regulations.
• Participate in delivering CSL Signature Service Programs to residents.
• Assist and provide input in updating UAIs, ISPs, and other assessments for the Sweet Memories Program.
• Participate in Care Plan meetings and family meetings.
• Communicate with families regarding residents’ involvement in the activity program.
• Assist in training staff for the Sweet Memories Neighborhood and Assisted Living activity programs.
• Provide guidance to staff on activity programs and resident quality of life.
• Participate in move-in processes and occupancy efforts.
• Assist in tours and customer contact to maintain occupancy.
• Coordinate special events and manage volunteers and transportation as needed.
• Be available for weekend coverage and capable of performing any job in the facility upon request.
