Job summary

The Meadows of Rockwell Retirement Center is seeking a compassionate Special Care Coordinator to lead Memory Care services, develop person-centered care plans and train staff. You will partner with the Executive Director, healthcare teams, residents’ families, and care staff to create a safe, engaging environment tailored to cognitive needs.

Qualifications

• Associate’s or Bachelor's degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years of experience in senior living, memory care, or healthcare.
• Supervisory or leadership experience in healthcare is desirable.
• Strong leadership, communication and interpersonal skills.

Responsibilities

• Lead Memory Care services with individualized care plans and staff training.
• Coordinate care with Executive Director, healthcare teams, families and care staff.
• Develop, implement and evaluate memory care programs for cognitive and emotional well-being.
• Ensure regulatory compliance and participate in quality improvement initiatives.
• Collaborate with nursing, physicians, therapists, and leadership to support operations.
