Springs of Ballentine is seeking a compassionate Special Care Coordinator to lead Memory Care services, develop individualized care plans, and train staff. You will partner with the Executive Director, healthcare teams, and families to support residents with cognitive impairments in a nurturing environment.

The role emphasizes collaboration, high-quality care, and adherence to dementia care best practices, with responsibilities spanning care planning, staff supervision, and program improvement.

Qualifications

• Associate’s or Bachelor’s degree in Nursing or Healthcare Administration preferred.
• Experience leading memory care or senior living teams is a plus.
• Strong communication and collaboration with families and care teams.

Responsibilities

• Develop, implement, and evaluate memory care programs focused on cognition, engagement, and well-being.
• Lead staff training to ensure memory care best practices.
• Serve as primary contact for residents' families regarding care needs.
• Ensure compliance with state regulations and quality standards for memory care.
• Collaborate with nursing, physicians, and therapists to coordinate care.
