Job summary

Springs of Ballentine is seeking a compassionate Special Care Coordinator to lead Memory Care services, focusing on individualized care plans, staff training, and resident engagement. This role partners with the Executive Director, healthcare teams, residents’ families, and care staff to support a safe, nurturing environment for individuals with cognitive impairments.

The ideal candidate holds an Associate’s or Bachelor’s in Nursing or Healthcare Administration, maintains an active license, and

Qualifications

• Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years of experience in a senior living, assisted living, memory care, or healthcare setting.
• Previous supervisory or leadership experience in a healthcare environment is highly desirable.
• Strong leadership, communication, and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of state regulations and compliance standards for assisted living and memory care.
• Proficiency in electronic health records (EHR) systems and basic computer skills.

Responsibilities

• Develop, implement, and evaluate memory care programs that promote cognitive function, social engagement, and emotional well-being.
• Provide leadership, coaching, and specialized dementia training to care staff, ensuring understanding of memory care principles.
• Serve as the primary point of contact for residents and families regarding care-related concerns; facilitate family meetings and updates.
• Ensure Memory Care program compliance with state regulations, company policies, and dementia care best practices; participate in quality audits.
• Collaborate with nursing, physicians, therapists, and other providers to coordinate care services.
