Job summary

HumanGood is seeking a Memory Support Program Manager for Rydal Park in Jenkintown, PA. This leadership role oversees the Memory Support neighborhood, resident experience, and day-to-day operations while guiding dementia programming and team development.

The role collaborates with nursing, life enrichment, rehabilitation, dining, and social services to deliver individualized resident plans, support families, and ensure compliance with community standards.

Qualifications

• Current Pennsylvania LPN license, Bachelor's or Master's in Social Work, or CTRS certification preferred.
• Minimum 2 years of experience working with older adults living with dementia or related cognitive impairments.
• Supervisory, management, or team leadership experience in healthcare, long-term care, memory care, or senior living preferred.
• Ability to educate, coach, and support families and staff regarding dementia care best practices.
• Strong communication, relationship-building, conflict-resolution, and problem-solving skills.
• Ability to collaborate effectively with residents, families, physicians, and interdisciplinary team members.
• Experience with electronic health records and documentation systems; PCC experience preferred.
• CDP, CDCM, or other dementia-related certifications are a plus.

Responsibilities

• Lead HumanGood's Immerse Program across the community.
• Supervise, coach, mentor, and develop team members with a culture of accountability and compassion.
• Coordinate with nursing, life enrichment, rehabilitation, dining, and social services for individualized resident support plans.
• Educate and support residents' families navigating the dementia journey.
• Serve as a resource on dementia care approaches and engagement.
• Monitor resident well-being and identify opportunities to enhance quality of life.
• Support development and implementation of meaningful programming tailored to residents' interests and cognitive needs.
• Participate in care conferences, interdisciplinary meetings, and family meetings.
• Ensure regulatory compliance, accurate documentation, and adherence to policies.
• Respond to concerns and resolve issues collaboratively.
• For licensed clinical candidates, perform functions within the scope of licensure.
