Job summary

The Alden Network strives to provide a patient-centered Memory Care Program on the Memory Care Unit. The Nurse supports a holistic, therapeutic approach and works to achieve the highest quality of life for residents with dementia.

This role emphasizes ongoing dementia care education, supervision of aides and CNAs, meticulous documentation, and collaboration with the inter-disciplinary team to deliver person-centered care on a regulated unit.

Qualifications

• Commitment to ongoing dementia care education and training.
• Ability to supervise Memory Care Aides and CNAs and coordinate care.
• Proficiency with documentation and care plan maintenance.

Responsibilities

• Support and assist in a holistic Memory Care Program on the Memory Care Unit.
• Oversee ADL care, transportation, and non-scheduled activities by staff.
• Maintain individualized symptom management tracking and care plans.
• Complete required documentation and ensure regulatory compliance.
• Engage families and support team communication.
