Job summary

The Hermitage is seeking a compassionate Special Care Coordinator to lead Memory Care services, focusing on individualized care plans, staff training, and resident engagement. You will partner with the Executive Director and healthcare teams to support a safe, nurturing environment for residents with cognitive impairments.

Qualifications

• Associate's or Bachelor's degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years of experience in a senior living, assisted living, memory care, or healthcare setting.
• Strong leadership, communication, and interpersonal skills.

Responsibilities

• Develop, implement, and evaluate specialized memory care programs that promote cognitive function, social engagement, and emotional well-being.
• Provide leadership, coaching, and dementia training to care staff, ensuring understanding of memory care principles.
• Serve as the primary contact for residents and families regarding care-related concerns; facilitate family meetings and care conferences.
• Ensure Memory Care program compliance with state regulations and company policies; conduct quality audits and drive continuous improvement.
• Coordinate with nursing, physicians, therapists, and leadership to support community operations and resident satisfaction.
