Job summary

The Hermitage in Sylva, NC seeks a dedicated Resident Care Coordinator to enhance resident well-being by coordinating care services and managing care plans. You will work with the Executive Director, clinical staff, and families to promote a safe, person-centered environment.

Responsibilities include developing ISP-driven care plans, supervising caregiving staff, engaging residents and families, and ensuring regulatory compliance and quality assurance.

Qualifications

• Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years in senior living, assisted living, memory care, or healthcare.
• Previous supervisory or leadership experience is highly desirable.
• Strong leadership and communication abilities; multitasking in fast-paced settings.

Responsibilities

• Care Plan Development & Management: Assess residents’ needs and create ISPs with clinical staff and families.
• Staff Supervision & Training: Lead caregiving staff and support scheduling for proper staffing.
• Resident & Family Engagement: Be the primary contact for care-related concerns; run family meetings.
• Regulatory Compliance & Quality Assurance: Ensure compliance with state regs and company policies.
• Collaboration: Coordinate with nursing, physicians, therapists, and leadership.
