Bay Harbor Post Acute and Healthcare Center is seeking a Memory Care Director who is a Maryland-licensed LPN. The role focuses on coordinating care for residents with dementia, supervising staff, and ensuring high-quality resident experiences.

The position is full-time with a day shift, offering competitive hourly pay and benefits. Responsibilities include coordinating admissions, collaborating with interdisciplinary teams, and maintaining compliant documentation.

Qualifications

• Must be a Maryland-licensed LPN.
• Experience in Skilled Nursing/rehabilitation settings preferred.
• Excellent communication, customer service, and interpersonal skills.
• Proficiency with Microsoft Office and EMR systems.

Responsibilities

• Serve as primary contact for residents and families on memory care unit; provide education and address concerns.
• Collaborate with nursing, therapy, social services, admissions, dietary, and other teams to meet resident needs.
• Assist with admission and discharge planning and follow-up coordination.
• Facilitate communication among residents, families, physicians, and team members for continuity of care.
• Conduct satisfaction rounds and help resolve concerns promptly and professionally.
• Support care conferences and interdisciplinary meetings to advocate for resident-centered care.
• Maintain accurate documentation and ensure compliance with policies and regulations.
• Assist with resident engagement initiatives to enhance overall experience.
• Perform other duties to support exceptional resident care and service.
