Bay Harbor Post Acute and Healthcare Center is seeking a Memory Care Director (LPN) for a full-time day shift in Salisbury, MD. The role oversees memory care services, coordinates with interdisciplinary teams, and supports admission/discharge planning to ensure compassionate, resident-centered care.

The position offers an hourly range of $31.00–$35.00 with incentives, plus benefits. Daily pay options and a no-frills option with higher hourly pay are available.

Formación

• Must be a Licensed LPN in Maryland.
• Experience in Skilled Nursing/Rehab/Long-Term Care or Assisted Living preferred.
• Excellent communication and interpersonal skills.
• Strong organizational skills with ability to manage multiple priorities.

Responsabilidades

• Serve as main contact for residents and families on memory care unit.
• Collaborate with nursing, therapy, social services, admissions, dietary, and other teams.
• Assist with admission and discharge planning and coordination of services.
• Facilitate communication among residents, families, physicians, and team members.
• Conduct resident and family satisfaction rounds and address concerns promptly.
