Reflections Retirement Community in Crystal Lake, IL is seeking an RN or LPN with dementia care leadership to design and manage a comprehensive Memory Care program, ensuring safety and resident well-being at Grand Victorian of Crystal Lake.

The role oversees budgeting, staffing, training, and quality initiatives, collaborating with families while maintaining compliance to deliver compassionate, dignified care that supports independence.

Qualifications

• Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members.
• Maintains current state RN or LPN licensure with Dementia Care Professional certification preferred.
• Minimum of three years experience in dementia care.
• Excellent written and verbal communication skills.
• Demonstrates interpersonal skills with love, compassion and dignity.

Responsabilités

• Designs and facilitates the Memory Care program with active participation from residents, families and team members.
• Oversees the budget and implementation of a cost-effective Memory Care program and resources.
• Creates a welcoming, safe environment promoting resident wellness and independence.
• Participates in move-in wellness assessments and individual care plans.
• Trains and leads the Memory Care clinical team, providing hands-on support and training.
• Delivers health promotion and dementia education for residents and families.
• Engages with residents and families to create a positive experience.
• Contributes to quality assurance and performance improvement initiatives.
• Coordinates infection control to maintain state regulatory compliance.

Job Details: Job Location: Grand Victorian of Crystal Lake - CRYSTAL LAKE, IL 60012, Position Type: Full Time, Salary Range: $75,000.00 - $80,000.00 Salary/year, Job Category: Management

Responsibilities

• Designs and facilitates the Community Memory Care program, promoting active participation from residents, family members, and team members
• Oversees the budget and implementation of a cost-effective Memory Care program, including managing an inventory list of equipment and supplies and maximizing resources available
• Creates and sustains a welcoming and safe environment which promotes resident wellness and independence
• Participates in the resident move-in process through initial wellness assessments and the development of individualized care plans
• Trains, schedules, and leads the Memory Care clinical team in providing exceptional care experiences, providing hands‑on support, education and training opportunities
• Delivers health promotion services and dementia care education services for residents and families
• Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
• Contributes toward quality assurance and performance improvement initiatives
• Coordinates the Community’s infection control program while maintaining state regulatory compliance

Qualifications

• Maintains current state Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure with Dementia Care Professional certification preferred
• Minimum of three (3) years experience in dementia care
• Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members
• Excellent written and verbal communication skills
• Demonstrates exceptional interpersonal skills and ability to serve withlove,compassion, anddignity
