Job summary

Gardant-Management-Solutions in Crystal Lake, IL is seeking a Nursing Leader to oversee its Community Memory Care program. The role emphasizes resident wellness, independence, and a welcoming, safe environment, with hands-on supervision of the clinical team.

You will conduct initial wellness assessments, develop individualized care plans, mentor staff, and coordinate infection control and regulatory compliance while managing budget and resources.

Qualifications

• State RN or LPN licensure with Dementia Care certification preferred.
• Minimum of 3 years in dementia care with leadership experience.
• Strong written and verbal communication; compassionate, patient-focused.
• Ability to mentor and motivate a clinical team.

Responsibilities

• Lead memory care clinical team with hands-on support and training.
• Develop individualized care plans during resident move-in and assessments.
• Manage budget and equipment inventory for cost-effective program.
• Coordinate infection control and ensure regulatory compliance.
• Deliver health promotion and dementia education to residents and families.
