Qualifications

Responsibilities

• Assist Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person-centered dementia-friendly programs.
• Engage residents in meaningful dementia care activities and document progress as required.
• Review individual resident service plans and ensure programs meet Brookdale standards.
• Coordinate and participate in community events and communications with residents, families, and staff.
• Support quality improvement initiatives and staff training related to programming.
