Brookdale is seeking a Clare Bridge Program Assistant to support person-centered dementia programs across our community. You will help plan and lead activities that enhance residents' quality of life and respect their routines and preferences, in alignment with Brookdale's care philosophy.

You will collaborate with the Program Manager or Coordinator, assist with documenting plans, coordinating events, and guiding volunteers, with opportunities for growth into leadership roles within the Clare

Qualifications

• High School Diploma or equivalent is required.
• Associates degree preferred.
• Experience with older adults and dementia preferred.

Responsibilities

• Support implementation of person-centered dementia programs.
• Assist in reviewing resident service plans and documentation.
• Coordinate special events and engage families.
• Provide guidance on programming quality and BEST standards.
