Job summary

Pacific Health Group in the United States is hiring a Member Engagement Specialist to be the first contact for prospective members. You will conduct outbound outreach, educate eligible individuals about programs, assist with enrollments, and support Medi-Cal applications, ensuring smooth transitions into ECM services.

Qualifications

• High school diploma or equivalent required.
• Associate’s or Bachelor's in social work, public health, healthcare administration, or related field preferred.
• 1–2 years in customer service, healthcare, or related field preferred.

Responsibilities

• Conduct outbound telephone outreach to prospective members referred by health plans and partners.
• Educate prospective members on Enhanced Care Management (ECM) and PHG services.
• Assist with enrollment and collect required consents and documentation.
• Assist with Medi-Cal and BenefitsCal applications when appropriate.
• Coordinate with Lead Care Managers to support transitions into ongoing care management.
