Pacific Health Group seeks a Member Engagement Specialist in California to drive outbound outreach, educate eligible individuals about ECM and PHG services, and assist with enrollments and Medi-Cal applications. You will build rapport with plans, providers, and community organizations, coordinate warm handoffs to care managers, and promote access to ECM and CS services.

The role requires strong communication, compassion, and organizational skills, with hybrid work and a schedule Mon-Fri.

Qualifications

• High school diploma or equivalent required.
• Associate's or Bachelor's in social work, public health, or related field preferred.
• 1–2 years experience in customer service, healthcare, outreach, or care coordination.
• Excellent verbal communication and interpersonal skills.
• Willingness to conduct outbound calls and travel for events (occasional).

Responsabilités

• Conduct outbound outreach to educate prospective members about PHG programs and ECM services.
• Assist with Medi-Cal and BenefitsCal applications when appropriate.
• Enroll members, obtain consents, and document interactions.
• Coordinate with Lead Care Managers for smooth transitions into ongoing care.
• Represent PHG at community events to promote services and awareness.
